Fop Lodge 43 membership is available to all Local, State and Federal active sworn or retired officers. Dues for membership are as follows:
Active Sworn Members: $20.00 per month membership dues. A $50,000 AD&D policy is available for an additional $5.00 per month.
Retiring Members (who have been in good standing for at least one year preceeding their retirement): $25.00 annual membership dues.

Upon receipt and review of your membership application, it will be processed and presented at our next scheduled monthly membership meeting for acceptence into the Lodge.

Membership Dues Payment Options
There are three payment options for members unable to participate in payroll deduction from their agency:
1) Automatic Monthly Credit Card Charge: Your designated credit card will be charged your selected monthly membership option on or about the 5th of each calendar month.
2) Automatic Monthly Bank Draft: Your selected monthly membership dues will be drafted directly from your designated bank account on or about the 10th of each calendar month.
3) Quarterly Billing: Quarterly invoices are sent at the beginning of each calendar quarter(January, April, July and October). Quarterly invoices are not prorated. Your first invoice will be for the quarter following the submission of your request. Invoices are due within two weeks of receipt.
Annual Billing: Annual Invoices are sent in January of each year. Annual billing requests submitted during the year will be prorated on a quarterly basis for the remainder of the first year and billed each January thereafter.
**Active members who retire during the year will be redesignated as a Retired Member and receive their annual invoice the following January. You DO NOT need to submit a billing request form**

For Credit Card or Bank Draft, download, print and complete the form, then send via mail to the address listed above. Due to the forms containing banking information, sending the forms via e-mail is not recommended. For Quarterly/Annual Billing, fill out and submit the online form.
